ABSTRACT

ObjectiveThis study aimed to predict the pharmacodynamic material basis and core targets of Bailing capsules in the treatment of chronic obstructive pulmonary disease (COPD) based on network pharmacology and molecular docking, which were further verified by cell experiments to explore the mechanism. MethodThe main active ingredients and related targets of Bailing capsules were screened in Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform (TCMSP) and SwissTargetPrediction. The main COPD targets were searched from GeneCards, DrugBank, Online Mendelian Inheritance in Man (OMIM) and Therapeutic Target Database (TTD). The protein-protein interaction (PPI) network was constructed by STRING and Cytoscape 3.6.1. Gene Ontology (GO) function annotation and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analysis were performed by the Database for Annotation, Visualization and Integrated Discovery (DAVID). Molecular docking verification was carried out using AutoDock Vina. The cell viability was detected by 3-(4,5-dimethylthiazol-2-yl)-5-(3-carboxymethoxyphenyl)-2-(4-sulfophenyl)-2H-tetrazolium (MTS) assay, and the mRNA level of the targets was detected by real-time polymerase chain reaction (Real-time PCR). ResultA total of 11 active ingredients of Bailing capsules such as cerevisterol, 270 related drug targets, and 1 020 COPD target proteins were obtained, with 74 intersection targets. The visualization analysis of the PPI network showed that the core targets of Bailing capsules in the treatment of COPD were tumor protein P53 (TP53), catenin beta 1 (CTNNB1), tumor necrosis factor (TNF), interleukin-6 (IL-6) and insulin (INS). Further, 20 signaling pathways were screened by KEGG enrichment analysis as the main pathways for Bailing capsules to treat COPD, involving phosphatidylinositol 3-kinase/protein kinase B (PI3K/Akt), cyclic adenosine monophosphate (cAMP), forkhead box O (FoxO), TNF, and hypoxia inducible factor-1 (HIF-1) signaling pathways. Molecular docking validation demonstrated that four active ingredients had stable binding to IL-6, with the lowest energy. Bailing capsules could reduce the mRNA level of IL-6 in RAW264.7 cells induced by lipopolysaccharide (LPS) (P<0.01) compared with the control group. ConclusionThe pharmacological mechanism of Bailing capsules in the treatment of COPD might be that its main active ingredients improved the inflammatory response by acting on TP53, CTNNB1, TNF, IL-6 and other targets and regulating PI3K/Akt, cAMP and other signaling pathways, thereby ameliorating COPD symptoms. This study provided experimental basis for subsequent in-depth research, and provided a diagnosis and treatment direction for disease-related clinical treatment.

ABSTRACT

This Meta-analysis was designed to evaluate the effects of Bailing Capsules on microinflammation and nutritional status of maintenance hemodialysis patients, and to determine its efficacy and safety. The randomized controlled trials concerning the intervention of microinflammation and nutritional status in maintenance hemodialysis patients with Bailing Capsules were searched from Chinese and English databases including CNKI, Wanfang, VIP, PubMed, EMbase, and Cochrane Library. A total of 16 articles were obtained, involving 1 095 cases. As revealed by Meta-analysis,(1)Bailing Capsules lowered the levels of serum high sensitivity C-reactive protein(SMD=-0.92, 95%CI[-1.05,-0.80], P<0.000 01), interleukin-6(SMD=-1.49, 95%CI[-1.96,-1.02], P<0.000 01), and tumor necrosis factor-α(SMD=-1.48, 95%CI[-1.68,-1.28], P<0.000 01) in patients with maintenance hemodialysis, thus alleviating microinflammation.(2)Bailing Capsules elevated the levels of serum hemoglobin(SMD=1.37, 95%CI[1.21, 1.54], P<0.000 01), albumin(SMD=0.78, 95%CI[0.57, 0.98], P<0.000 01), and triglyceride(SMD=0.29, 95%CI[0.07, 0.50], P=0.01) in patients with hemodialysis to improve their nutritional status.(3)Bailing Capsules reduced the incidence of cardiovascular events(RR=0.45, 95%CI[0.34, 0.59], P<0.000 01).(4)A total of six patients presented with mild gastrointestinal discomfort after receiving Bailing Capsules, and no serious adverse reactions were observed. The sequential analysis showed that the sample size of this Meta-analysis had reached the expected value. Meanwhile, the grade of evidence quality suggested that the outcome indicators were mainly low or extremely low in quality. In conclusion, Bailing Capsules might have potential advantages in alleviating microinflammation, improving nutritional status, and reducing the incidence of cardiovascular events. However, in view of the low quality and evidence of the included literature, high-quality clinical trials are needed to further confirm the efficacy and safety of Bailing Capsules.

ABSTRACT

OBJECTIVE:To study the efficacy and safety of sacubitril valsartan sodium tablets combined with Bailing capsules in the treatment of chronic left heart failure with renal insufficiency ,and to provide reference for clinical drug use. METHODS : Totally 96 patients with chronic left heart failure with renal insufficiency who sought medical care in our hospital from Nov. 2018 to Nov. 2019 were divided into group A ,B and C according to table of random numbers ,with 32 cases in each group. Group A received conventional heart failure treatment and and Bailing capsules (2 g each time ,3 times a day );group B received conventional heart failure treatment and Sacubitril valsartan sodium tablets (50 mg each time ,twice a day );group C was given with heart failure treatment and Sacubitril valsartan sodium tablets (50 mg each time ,twice a day )and Bailing capsules (2 g each time,3 times a day ). 3 groups received consecutive 6 months of treatment. Clinical response rates of 3 groups were compared. Left heart function indexes [left ventricular end systolic diameter (LVESD),left ventricular end-diastolic diameter (LVEDD),left ventricular ejection fraction (LVEF)] and serological indexes [interleukin 1(IL-1),IL-6,N terminal brain natriuretic peptide precursor,glomerular filtration rate (GFR)] were detected before and after treatment. The occurrence of ADR were observed and recorded. RESULTS :During this study ,a total of 6 patients fell off ,and eventually 90 patients completed the study ,including 29 cases in group A ,30 cases in group B and 31 cases in group C. Before treatment ,there was no statistical significance in left heart function indexes or serological indexes among 3 groups(P> 0.05). After 6 months of treatment ,clinical response rate of group C was significantly higher than those of group A and B 163.com (P<0.05). Compared with before treatment , LVEDD, LVESD and serological indexes of 3 groups were decreased significantly after treatment (P<0.05),while LVEF and GFR were increased significantly (P<0.05);the changes of above indexes (except for IL- 1 level in serum ) in group C were significantly better than group A and B ,the changes of above indexes in group B (except for GFR )were significantly better than group A (P<0.05). No significant ADR were observed in 3 groups. CONCLUSIONS :Sacubitril valsartan sodium tablets combined with Bailing capsules can significantly decrease the level of serum inflammation factors ,and improve cardiac and renal function in patients with chronic left heart failure with renal insufficiency ,with good safety.

ABSTRACT

Objective To observe and compare the effect of Cordyceps sinensis ( bailing ) capsules com-bined with weight-bearing breathing exercises, and weight-bearing breathing exercises combined with tiotropium bro-mide and seretide, on patients with stable but moderate-to-severe chronic obstructive pulmonary disease (COPD). Methods Sixty-three patients with moderate-to-severe COPD were randomly divided into an observation group and a control group. Both groups performed weight-bearing breathing exercises, supplemented in the observation group with the oral administration of bailing capsules. The control group instead inhaled tiotropium bromide and seretide. Six-mi-nute walking distance, the COPD assessment test ( CAT scores) and concentrations of interleukin-6 ( IL-6) , interleu-kin-8 ( IL-8) and tumor necrosis factor-α ( TNF-α) were observed after 1 ( T1) , 30 ( T2) and 58 ( T3) days of the treatments. Results At T1 there were no significant differences between the two groups in any of the measurements ( P≤0.05) . At T2, there were still no significant differences except that a significant decrease in IL-8 and TNF-αlevels was observed in the control group. At T3 the average CAT scores had decreased significantly in both groups compared to before the treatment but there was no significant difference between the two groups. In the observation group, the average 6MWT distance had increased significantly compared to before the treatment and compared to the control group, where there was no significant improvement. The average IL-6, IL-8 and TNF-αreadings of the control group were significantly lower than those of the observation group at T3 and compared to before the treatment. No sig-nificant changes in those indicators were observed in the observation group at T3. Conclusions Bailing capsules combined with weight-breathing exercises are more effective for relieving dyspnea symptoms and improving exercise capacity than weight-breathing exercises combined with tiotropium bromide and seretide. However, in controlling air-way inflammation and airway hyper-responsiveness, the triple inhalation combined with weight-bearing breathing exer-cises is more effective.

ABSTRACT

Objective:To observe the effect of Bailing capsules adjuvant therapy on urinary monocyte chemoattractant protein-1( MCP-1), microalbuminuria(mAlb) and 24h urinary protein(Up)in the children with henoch-schonlein purpura nephritis(HSPN). Methods:Totally 64 cases of HSPN children were randomly divided into two groups with 32 cases in each .The normal group was given the conventional hormone , dipyridamole and loratadine etc .The Bailing group was orally treated with Bailing capsules additionally .The therapy course was 4 weeks, the changes of MCP-1,mAlb and 24hUp in the groups before and after the treatment were observed .Re-sults:Before the treatment, the levels of MCP-1,mAlb and 24hUp were similar between the groups (P>0.05).After the 4-week treatment, the levels of MCP-1,mAlb and 24hUp were lower than those before the treatment (P<0.05),and the decrease in Bailing group was more significant than that in the normal group (P<0.05).Conclusion:Bailing capsules adjuvant treatment for HSPN can significantly reduce albuminuria and improve renal function .

ABSTRACT

Objective:To investigate the effects of Bailing capsules on the expression of TGF-β1 in the peritoneal dialysis solution for peritoneal dialysis patients. Methods: Totally 40 patients treated with peritoneal dialysis ( PD) were randomly divided into two groups (20 cases in the control group and 20 cases in the experimental group). All the patients were treated with PD by 1. 5% perito-neal dialysis effluent (6 000 ml everyday), and the patients in the experimental group were additionally treated with Bailing capsules (5 capsules each time, three times a day after meals) for 6 months. The adequacy of PD (including Kt/V and Ccr) of the two groups was examined after the one-month treatment. The renal function and the level of TGF-β1 in the effluent in 1, 3 and 6 month were com-pared between the two groups. Results:The adequacy of PD ( Kt/V and Ccr ) had no significant difference between the two groups (P0. 05). In the control group, the expression of TGF-β1 in the effluent in 1, 3 and 6 month was increased gradually with significant difference (P0. 05), which was lower than that in the control group (P<0. 05). Conclusion:Bailing capsules can decrease the expression of TGF -β1 in the effluent for the patients treated with PD and inhibit the peritoneal fibrosis.

ABSTRACT

Objective:To explore the effects of Bailing capsules combined with salvianolate on inflammatory cytokines, renal vas-cular endothelia and renal function in diabetic nephropathy inⅣphase. Methods:Totally 60 patients with diabetic nephropathy in IV phase were randomly divided into the control group (30 cases) and the treatment group (30 cases). The control group was given the conventional and salvianolate therapy, and the treatment group was treated with Bailing capsules additionally. The treatment course was 3 months. The changes of serum creatine(SCr), urean itrogen (BUN), creatinine clearance rate (Ccr), ethelin (ET-1), interleukin-6 (IL-6), tumor necrosis factor (TNF-a) and C-reactive protein (CRP) were detected before and after the treatment. Results:After the 3-month treatment, SCr, BUN, IL-6, TNF-a, CRP and ET-1 were significantly decreased in the two groups with statistical signifi-cance, and Ccr was significantly increased in the treatment group (P0. 05). There was no significant difference in the incidence of adverse drug reactions between the two groups (P>0. 05). Conclusion:The combination of Bailing capsules and salvianolate can re-duce the inflammatory reaction and improve renal vascular endothelial function and renal function in the patients with diabetic nephrop-athy in Ⅳ phase with promising safety and few adverse reactions.

ABSTRACT

Objective:To investigate the clinical effects of dermatophagoides farinae drop combined with Bailing capsules in the treatment of allergic rhinitis. Methods:Totally 96 patients with allergic rhinitis in our hospital were selected and randomly divided into two groups with 48 cases in each by a randomization table. The patients in the treatment group received dermatophagoides farinae drop and Bailing capsules,and those in the control group only received dermatophagoides farinae drop. The scores of symptoms and physical signs and adverse reactions were compared between the two groups. Results:After the treatment,the scores of symptoms and physical signs in the two groups were significantly decreased(P 0. 05). Conclusion:Bailing capsules combined with dermatophagoides farinae drop can significantly relieve the clinical symptoms and signs,and improve the quality of life of the patients with allergic rhinitis,which is worthy of promoted application in clinics.

Objective:To observe the efficacy and safety of azithromycin combined with Bailing capsules in the treatment of chron -ic obstructive pulmonary disease ( COPD) in stable phase .Methods:Totally 154 cases of COPD patients were randomly divided into the treatment group(80 cases)and the control group(74 cases).Both groups were treated with the routine symptomatic treatment , and at the same time, the control group was treated with Bailing capsules (1 g, po, tid)and the treatment group was treated with azithromy-cin tablets (250 mg, po, qd)combined with Bailing capsules(1 g, po, tid).The course of treatment was 6 months and all the patients were followed up for 6 months after the treatment.The clinical efficacy, lung function, exercise tolerance, quality of life, the times of acute exacerbation and the incidence of adverse drug reactions of the two groups were observed and compared .Results:After the treat-ment, the lung function, exercise tolerance and quality of life were improved significantly in the two groups (P0.05).Conclusion:Azithromycin tablets combined with Bailing capsules can improve lung function , enhance exer-cise tolerance , improve the quality of life and reduce the frequency of acute exacerbation effectively with good security in the treatment of patients with COPD in stable phase .

ABSTRACT

Objective:To explore the therapeutic effect on blood glucose, lipid, and renal function of Bailing capsules combined with calcium dobesilate dispersible tablets in the treatment of diabetic nephropathy. Methods: Totally 216 patients with diabetic ne-phropathy in phase Ⅳ were randomly divided into the control group (108 cases) and the treatment group (108 cases). The control group was given the conventional therapy, and the treatment group was treated with Bailing capsules combined with calcium dobesilate dispersible tablets additionally. The levels of serum creatine (Scr), urean ntrogen (BUN), endothelin (ET-1), β2-microglobulin (β2-MG) ,cystatin C ( CYC) and urinary albumin excretion rate ( UAER) were detected for all the patients before and after the treat-ment. The adverse drug reactions were observed as well. Results:After the treatment, all the indices in the two groups were all obvi-ously decreased (P0. 05). The adverse drug reactions showed no significant differences between the two groups (P>0. 05). Conclusion:The effect of Bailing capsules combined with calcium dobesilate dispersible tablets is better than that of calcium dobesilate dispersible tablets in the treatment of diabetic nephropathy in phase IV, which can improve the renal function indices with high safety and without notable influence on blood glucose and lipid.

ABSTRACT

Objective To explore the preventive effect of Bailing capsules(BL) on myocardial fibrosis in the viral myocarditis(VMC) mice models and to clarify the possible mechanism.Methods The models of VMC were established by injecting Coxsackie virus B3 (CVB3) solution into 50 BALA/C mice.The 31 mice were divided into model group(n =11),large dose BL group(n =10)and small dose BL group (n =10).At the same time the control group (n =10) was established.The mice in model group and control group were fed with water 1 mL/d,the mice in large dose BL group were fed with BL at a dose of 7.5 g/(kg · d),and the mice in small dose BL group were fed with BL at a dose of 2.5 g/(kg · d).After 60 d,the levels of angiotensin Ⅱ (Ang Ⅱ) in blood serum of mice in various groups were detected by using ELISA.Myocardium tissue of mice was stained by Masson and collagen volume fraction(CVF)was accounted.At the same time,the content of periostin mRNA and transforming growth factor β1 (TGF-[β1) mRNA in myocardium tissue of mice were detected by adopting reverse transcription(RT)-PCR.Results Compared with the control group,the levels of serum Ang Ⅱ,CVF,TGF-β1 mRNA and periostin mRNA were increased (all P < 0.01) in the model group.Compared with the model group,the levels of serum Ang Ⅱ,CVF and TGF-β1 mRNA were obviously decreased in large dose BL group and small dose BL group (all P < 0.05).Compared with the model group,the level of periostin mRNA was obviously decreased in large dose BL group (P < 0.05),but the level of periostin mRNA in small dose BL group was not decreased obviously (P > 0.05).Conclusions BL can offer some protection to myocardial fibrosis in VMC mice.The possible mechanism may be performed through inhibiting the expression of serum Ang Ⅱ and TGF-β1 in myocardium tissue to reduce the expression of periostin.

ABSTRACT

Objective:To study the clinical effect of Bailing capsules in the treatment of diabetic impotence. Methods: Totally 136 cases of diabetic impotence were randomly divided into the observation group and the control group with 68 cases each. The obser-vation group was treated with Bailing capsules and the control group was with tadalafil tablets. The effect of the two groups was com-pared, and the changes in IIEF-5 score, serum testosterone ( T) and zinc ( Zn2+) before and after the treatment were studied as well. Results:The total effective rate of the observation group was 83. 82% while that of the control group was 67. 65%, and the difference was statistically significant (P0. 05), while IIEF-5 score was increased significantly (P0.05), while the serum testosterone (T) and IIEF-5 score were increased significantly in the observation group (P <0. 05). After the treatment, the symptoms in the two groups were both improved with increased IIEF-5 score, T and Zn2+, and the ob-servation group was with more obvious improvement. Conclusion:The effect of Bailing capsules in the treatment of diabetic impotence is promising, which is worthy of further study.
